Aspire Foundations: Your First Six Months

If the care feels like a good fit, you’ll move into the Aspire Foundations program.

Your health deserves time, attention, and coordination with the best clinicians. The first 6 months is when we really get that work done. You’ll receive high-touch, deeply invested care with your Nurse Practitioner.

Clinical care

• Four one-hour visits with your nurse practitioner

Collaboration

• Two health coaching visits

• When your plan would benefit from collaboration, we cover the initial consult with our trusted partners, such as pelvic floor therapy, dietitian, ADHD coach, sexuality coaching

Day-to- day Support

• Strength training app membership

• Unlimited messaging with the team between visits

• Continuous glucose monitor

• Mira Hormone testing, when indicated

• Quarterly office hours

After Six Months: Choose Your Membership

By month six, your foundation is built. Membership ensures you keep moving in the right direction. Membership is month to month, billed quarterly.

• One 60-minute visit with your NP each quarter

No. We are a specialized concierge practice that complements your primary care provider. We strongly recommend you maintain a primary care relationship and we will coordinate directly with your PCP whenever helpful.

Membership includes extended visits, direct messaging access to your clinician, careful review of your records and labs, coordinated lab orders, and detailed plans after each visit. Member perks also include a welcome gift, Office Hours with expert clinicians, discounts on supplements, educational opportunities and more!

Yes. Our beautiful clinic is in downtown Northfield, MN. We offer in-person services, gynecologic care, and procedures such as IUD insertions, endometrial biopsies, vulvar biopsies, and more.

We are a private-pay practice. We do accept HSA & FSA. We can provide a superbill you may submit to your insurance for possible out-of-network reimbursement.

Labs, medications, and other testing can be submitted to your insurance..
